Discharge: stay healthy!

Is your stay at the Schüchtermann-Klinik already coming to an end? If so, you can find important information here about your discharge.

 

For patients in our cardiology department

As a rule, you will be discharged after the ward round around noon. The ward round also usually includes the final discussion.

Also around noon you will receive your written medical discharge report. Please take this report to your family doctor, who will then prescribe any changed medication. There may be a waiting time between the ward round and the preparation of the discharge report, which you are welcome to spend in our cafeteria. For this you will be given coffee tokens by our nursing staff. 

If you are discharged on a Wednesday or at the weekend and your family doctor cannot be reached, you will be given the medication and a medication chart by our nursing staff.

Certificates confirming that you are unable to work, certificates for hospital daily benefits (if desired), pacemaker or Marcumar (phenprocoumon) identification cards are always attached to the discharge documents.

On the day of discharge you can take all of your meals up to the time of your discharge from the clinic. If necessary, family members can eat lunch with you in the dining room after they have purchased a meal ticket. 

If you are a patient receiving acute inpatient treatment in the cardiology department and this is being paid for by the statutory health insurance, please go to the Patient Administration Department on the ground floor, room E 75, to clarify the additional amount to be paid by you. Payment in cash or with a debit card is possible.

For patients in our cardiosurgical department

On the day of your discharge you will be handed the discharge letter in the surgical ward for your family doctor and/or the clinic performing further treatment. In the case of a standard discharge procedure, please vacate your room between 9 and 10 a.m. If no one is able to collect you, the reception will be pleased to order a taxi for you.  

If you are a patient receiving acute inpatient treatment in the surgery department and this is being paid for by the statutory health insurance, please go to the Patient Administration Department on the ground floor, room E 75, to clarify the additional amount to be paid by you. Payment in cash or with a debit card is possible.

For patients in our rehabilitation department

Before you leave, some final examinations (in accordance with the therapeutic scheme) and a final discussion with your ward doctor will be held. You can see the dates of these on a notice on the bulletin board in the corridor of your ward.

The ward doctor will discuss with you whether and – if applicable – when you can resume your profession, which medications you must continue to take in the future and whether you should continue with exercise therapy in an outpatient cardiac group

The most important information will be given to you by the ward doctor in a condensed provisional report for your family doctor. The detailed discharge report will be sent to your family doctor somewhat later. Please ask your ward doctor or nurse to return to you the medical documentation you brought for admission. Before you leave, please hand in your therapeutic scheme and all measurement records to the nurses' room on your ward.

If your rehabilitation treatment is to be paid for by a statutory health insurance company, you will have to pay a personal contribution, including the previous period of acute care, for a maximum of 28 days at €10 a day. Please inform yourself during the course of your inpatient treatment in room E 74 on the ground floor about the level of your own contribution and the possible ways of paying. 

If you are travelling home by train, please contact the reception a few days beforehand to arrange your baggage collection and other formalities. Please vacate your room on the day of departure by approximately 8 a.m. and hand your room key in to reception.
